1. How are Evergreen At Home caregivers selected?
The caregivers are carefully screened by process of a personal interview, criminal background check, driving record check and insurance verification, and reference check. All caregivers receive extensive training with Evergreen’s nurse educator and Evergreen At Home Team Leader
2. Will I have the same caregiver for each visit?
If you request consistency with having the same caregiver, Evergreen At Home will strive to fullfill your request. If your caregiver is ill or on vacation, another caregiver will be specifically chosen to meet your needs. Evergreen At Home believes in building and nurturing relationships between clients and caregivers to allow for meaningful interactions.
3. What is the difference between Homemaker Services and Personal Caregiver Services?
Homemaker Services assist you with tasks such as housekeeping, laundry, personal errands, grocery shopping, meal preparation, and pet care.
Personal Caregiver Services assist you with activities of daily living such as dressing, bathing, grooming, hygiene, incontinence care, and medication reminders. Evergreen At Home cannot administer medications.
4. How is Evergreen At Home different from other home care agencies?
Evergreen At Home is a provider of home care services for adults age 55 and older. Services are brought to you by Evergreen, provider of outstanding care and programs for more than 40 years. Evergreen At Home clients enjoy in-home servicves as well as additional services the Evergreen campus has to offer, such as social programs and activities, wellness programs, art programs, and spa bathing services.
5. Why should I choose Evergreen At Home?

6. Will I need to sign a contract or agreement for services?
Yes, you will need to sign an agreement for services. This agreement states the following: type(s) of service you will receive; cost of service(s); how and when to provide a cancellation notice; payment terms; non-solicitation/substitution policy; and other additional terms and conditions.
7. Are gift certificates available for Evergreen At Home services?
Yes, gift certificates are available for any denomination. If interested, contact the marketing department at (920) 237- 2154.
8. Is becoming a client of Evergreen At Home going to guarantee my admission to Evergreen should I chose to move to the campus or my needs become such that I need additional services?
No, being a client of Evergreen At Home does not guarantee admission to Evergreen. A comprehensive assessment would have to be completed for residency on the Evergreen campus.
9. What other services does Evergreen At Home provide?
Evergreen At Home provides a 24-hour emergency response system called FirstCall; heavy housekeeping services; bathing services; and companion services. If you have personal caregiver services or homemaker services you may also be eligible to access home maintenance services; transportation services; respite services; check-in visits; vacant home watch; travel companion; and outpatient therapy services on the Evergreen campus.
